CareSTL Health has an immediate opening for a Health Home RN Care Manager.  The RN Care Manager under administrative direction, coordinates and integrates a continuous and comprehensive interdisciplinary plan of care for patients utilizing a holistic and patient-centered medical home approach;  assesses,  plans, implements, monitors, documents,  and evaluates the continuum of care in collaboration with primary care providers and the health care team;  facilitates options and services to meet the patient's health care needs including access to care, engagement, and education in self-care, and care coordination with community services and family members;  develops and implements patient centered strategies to improve management of chronic conditions, improve clinical outcomes, and enhance patient satisfaction. 

 

ESSENTIAL FUNCTIONS:

The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned.  The RN Care Manager may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons. Other duties may be assigned.

  • Consults with Community Support Staff about identified health conditions, facilitates health education groups and develops wellness and prevention initiatives
  • Participate in the initial treatment plan development for all of their Primary Care Health Home Enrollees as well as assist in developing treatment plan health care goals for individuals with co-occurring chronic diseases
  • Assist in contacting medical providers and hospitals for admission/discharge and in coordinating activities related to external referrals for assigned Health Home Enrollees
  • Conduct medication reconciliation at patient encounter to insure patient safety and treatment compliance as well as in-home visits as needed to improve patient self-management and treatment plan adherence in addition to assessments and screenings as indicated by enrollee age, gender, and health history
  • Utilize CareSTL Health's Health Information Technologies (HIT) tools and reports for treatment to guide care coordination and clinical judgment required by the Meaningful Use Initiative

Additional Functions:

  • Monitor and report performance measures and outcomes

Qualifications:

Education

  • High School Diploma/GED
  • Bachelor's Degree in Nursing, preferred
  • Graduate of an accredited school of Nursing required
  • Valid registration as a Registered Nurse in the State of Missouri
  • Current AHA Healthcare Provider training (BCLS)
  • Current CPR Certification required

Experience

  • 3 years experience in clinical nursing
  • Experience in case management strongly desired

Other

  • Must have reliable transportation

  • Strong proficiency in use of personal computer software and databases, including the use of electronic health records (EHR) and manipulation of statistical data
  • Demonstrated ability to work autonomously and be directly accountable; demonstrated ability to function effective in a rapidly changing environment; ability to prioritize, delegate, and organize work; ability to maintain a high level of confidentiality
