CareSTL Health seeks multiple full-time Registered Nurse Case Managers to develop, implement, and evaluate individualized patient care plans. They act as social workers, advocate patient welfare, and serve as a liaison between patients, their families, and healthcare providers. 



The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned.  The Registered Nurse Case Manager may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons. 

  • Review treatment goals and resources to evaluate patient and determine level of care; when the patient needs further, home planning, action plans and complete an expanded assessment of patient at time of initial/new clinic encounter
  • Communicates with physicians and other agencies and service providers to develop working relationship that facilitate case coordination and information sharing, while also facilitating investigations as well as medical and treatment referrals
  • Interpret medical documents while also helping patients understand their conditions and treatment options in addition to the evaluation of patient satisfaction and quality of care provided
  • Coordinate the integration of social services/case management function while ensuring that patient tests are appropriate and necessary, while maintaining appropriate cost
  • Participate in the development and implementation of patient care policies and protocols, while promoting effective and efficient utilization of clinical resources

Additional Functions:

  • Refer cases where patients and/or family would benefit from counseling and help patient and family make informed decisions
  • Provide education and literature about diagnosis
  • Actively demonstrate a working knowledge of nursing theories, techniques, principles and practice in order to holistically care for the patient
  • Any additional job duties as requested.


  • Graduate of an accredited School of Nursing
  • Valid registration as a Registered Professional Nurse in the State of Missouri
  • Current AHA Healthcare Provider training (BCLS)


  • 1-2 years experience working in Community Health Center/or Hospital setting

Transportation Requirement:

  • Must have reliable transportation

 A Successful Candidate:

  • Knowledge of information and techniques in providing excellent patient care
  • Knowledge of administrative and office procedures and systems such as electronic transmission via facsimile and/or  internet
  • Good computer skills including the use of office procedures and terminology
  • Good self motivational and organizational skills
  • Ability to maintain confidentiality
  • Ability to work all health center sites
  • Understanding of the complexities of integrating services
  • Knowledge of data management through use of IT
