CareSTL Health is seeking a Clinical Document Reviewer who is professional and competent in capturing documentation necessary to fully communicate patients' health status and conditions, who assumes responsibility for analyzing the medical record to provide feedback on documentation and recommendations for improved documentation. 


  • Reviews medical records for completeness and accuracy to include issuing citations to clinical staff when they are not and following up with staff to ensure record is in compliance.
  • Complete biweekly reports (chart audits and delinquent chart, encounters and lab reports).
  • Reviews and becomes thoroughly familiar with documentation in order to correctly interpret data according to the regulations.
  • Coordinate in-service meetings to educate the medical staff on proper documenting procedures.
  • Maintains current and accurate lists of records requested, pending and completed.


  • Supports compliance efforts by promoting a Code of Ethics for clinical documentation



  • High School Diploma or GED required
  • Associate or Bachelor degree in Health information or related field preferred


  • Previous health information management experience preferred
  • CCDS or CDIP certification preferred


  • Must have reliable transportation

A Successful Candidate:

  • Completed coursework in Medical Terminology, Anatomy and Physiology
  • Must be able to organize and work efficiently
  • Must be able to pay close attention to detail
  • Must be able to work with others
  • Has good interpersonal communication skills
  • Must have good knowledge of the MS Office Suite, including Outlook and Word
