CareSTL Health seeks full-time Licensed Practice Nurses (LPN) responsible for rendering nursing care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the registered nurse.


  • Evaluates patient to determine level of care, when the patient need further, home planning and other needs and action plans
  • Prepares and administers medications and treatments with knowledge of purpose and adverse effects of nursing care provided
  • Assists with providing patient care: records vital signs; discusses and records chief health concerns; performs tests; administers injections
  • Places phone calls to pharmacies to order prescriptions; sets-up referral contacts
  • Completes expanded assessment of patient at time of initial/new clinic encounter
  • Facilitates investigations, medical and treatment referrals
  • Helps patients understand their conditions and treatment options
  • Interprets medical documents
  • Ensures that patient tests are appropriate and necessary
  • Coordinates the integration of social services/case management function
  • Evaluates patient satisfaction and quality of care provided
  • Communicates with physicians and develop working relationship
  • Maintains appropriate costs
  • Refers cases where patients and/or family would benefit from counseling
  • Helps patient and family make informed decisions
  • Reviews treatment goals and resources
  • Provides education and literature about diagnosis
  • Participates in the development and implementation of patient care policies and protocols
  • Promotes effective and efficient utilization of clinical resources
  • Assists with daily preparation of exam rooms and by securing supplies and equipment
  • Explains procedures and treatments to patient to gain cooperation, understanding, and allay apprehension
  • Administers prescribed medications and treatments in accordance with scope of practice
  • Prepares equipment and aids Physician during treatment and examination of patient
  • Maintains awareness of comfort and safety needs of patient


A Successful Candidate:

  • Knowledge of information and techniques in providing excellent patient care
  • Knowledge of administrative and office procedures and systems such as electronic transmission via facsimile and/or  internet
  • Good computer skills including the use of office procedures and terminology
  • Good self motivational and organizational skills
  • Ability to maintain confidentiality
  • Ability to work all health center sites
  • Understanding of the complexities of integrating services
  • Knowledge of data management through use of IT



  • Education:  Graduate of an accredited school of Nursing required. Valid registration as a Licensed Practical Nurse in the State of Missouri required. Current AHA Healthcare Provider training (BCLS) required.
  • Experience: 1-2 years of experience in a community health center and/or hospital setting
  • Other:  Must have reliable transportation.
