CareSTL Health seeks a Clinical Pharmacist to serve as a clinical team leader (clinical pharmacy administrator) to manage, collaborate and support other staff pharmacists in providing Medication Therapy management (MTM), Medication Therapy Services (MTS) or any other clinical program initiated by the health center. The clinical pharmacist also assists in non-clinical pharmacy activities on a daily basis. 

Duties:

  • Reviews and verifies all new and refill prescriptions with accuracy and in a timely manner
  • Provides patient counseling and education as required by Missouri law
  • Communicates and collaborates with physicians, nurses, patients, care-givers and other team member to provide quality care for the patients
  • Performs medication therapy management (MTM) for eligible patients
  • Initiate and implement medication therapy services (MTS) program for patients who see primary providers at the clinic
  • Monitors safety and efficacy of drug therapy
  • Monitors outcomes of clinical programs (MTM and MTS)
  • Ensures proper record keeping and storage of prescriptions copies, drug order invoices and other pharmacy-related records required by state and federal laws
  • Ensures pharmacy staff understands and complies with Pharmacy and the Health centers' policy and procedures
  • Complies with state and federal drug laws as regulated by the state Board of Pharmacy
  • Monitor and manage inventory accordingly order and stock medications in the pharmacy
  • Maintain up-to-date pharmacotherapy knowledge and clinical skills
  • Maintain professional attitude and image to patients, multi-disciplinary care team and pharmacy staff

Qualifications:

  • BS and/or PharmD degree in Pharmacy. Graduate from an accredited school of Pharmacy required. Graduated from a Pharmacy Residency Program (PGY1 or PGY2) (preferred)
  • Missouri Pharmacist license (required)

 

Experience:

  • 1-2 years experience as a clinical pharmacist in ambulatory care, managed care, consultant or retail setting with direct patient interaction (required)
    • Other:  Must have reliable transportation.
